Application for limited-entry Landowner Association Certificate of Registration
Primary form for the Landowner Association president
The Utah Landowner Association Rule (administrative rule R657-43) was updated in December 2022. Please review the rule before submitting this application.
To apply for a COR for your Landowner Association, you must submit the following:
- Landowner Association primary form: The form below must be signed and submitted by the Landowner Association president.
- Signature form: All participating landowners in the Landowner Association (including the Landowner Association president, if the president owns any enrolled land) must enter landowner details, agree to the Landowner Association terms and submit their signature.
- A digital parcel map of the Landowner Association property: Map files may be uploaded with the primary form below in Esri .shp, .kmz, .kml or .gpx format (maximum file size 25 MB). Map files must include individual polygons depicting boundaries of each enrolled parcel. Each parcel must have attribute data that clearly identifies the parcel ID (which must match the parcel IDs on the spreadsheet table).
- A spreadsheet table of the enrolled properties with separate columns indicating each county parcel number, parcel map ID (which must match the parcel IDs on the digital map) and landowner name; also uploaded with the primary form in Microsoft Excel .xlsx or .xls format. - Governing documents: All items listed in rule R657-43-5(3)(b) must be clearly identified and uploaded with the primary form in .pdf, .docx, or .doc format (maximum file size 25 MB).
- $10 application fee: Contact your regional DWR office to pay over the phone. (Tell them the Landowner Association name and that you are paying an application fee.)
If you are not the Landowner Association president and only need to enter landowner details, agree to the Landowner Association terms and submit a signature as a participating landowner, skip to the signature form.
Once the primary form and all signature forms are received, a $10 nonrefundable processing fee will need to be paid at your DWR regional office. If the application is approved by the Utah Wildlife Board, the Landowner Association president will be invoiced a $450 COR fee, which is good for three years. The entire COR fee must be paid before any vouchers can be issued.
All applications for the following calendar year are due Sept. 1.
Primary form
Each participating landowner must agree to the provisions of the Landowner Association application and submit their signature in a separate form. Once all landowners' signatures have been received, the COR application will be complete.
